FEAT of WA Surface Giveaway – Terms & Conditions
Eligible applicants must have a child with an official autism diagnosis* ages birth up through 18 years of age, be a Washington state resident and have financial need. All applicants may be requested to provide documented proof on all eligibility requirements.
Applications must be submitted by parent or legal guardian on behalf of the person with autism. All applications will be reviewed and additional information may be required to verify eligibility. All applicants selected or not will be contacted via email on April 20, 2016. If selected, the Surface will be shipped via FedEx directly to the address provided on the application. Only one application per child with autism will be required to enter. This is based on eligibility and on a first come first served basis.

Surfaces will be shipped to the recipient unless other arrangements are made. FEAT may also offer an in person pick up option at our Bellevue office, on date selected (Applicant/photo ID will be required if picking up in person).
FEAT of WA is to be held harmless and is not liable for any damage, harm, injury, death or loss to persons or property as a result of using the Surface. If the Surface is lost, stolen, damaged or destroyed FEAT of WA is not responsible for replacement or repair, this is a one off give. FEAT of WA is not responsible for set-up, install or understanding of how to use a Surface. For all further information on how to use the product, please refer to the manual provided with the Surface. *Official autism diagnosis: Must have a medical diagnosis from physician or psychologist who is qualified to administer specific behavioral assessments, symptoms and diagnostic tests. Education evaluations are not accepted.
